The genetic basis of FCHL is not well understood, and probably several different genes can cause a similar phenotype (4). Hepatic overproduction of VLDL is a common metabolic basis of this condition. FCHL has been estimated to occur in approximately 1 in 200 persons overall, and about one in five patients with CHD under the age of 60. FCHL is suggested by the presence of a mixed hyperlipidemia with fasting triglyceride levels of 200–800 mg/dL, cholesterol levels of 200–400 mg/dL, and decreased HDL-C levels in the absence of secondary causes of hyperlipidemia.

Multiple different mutations in the gene encoding for CETP have been reported to cause CETP deficiency including a 5 donor splice site intron 14 G-to-A substitution and a missense mutation in exon 15 (D442G) (37,38). Individuals homozygous for the intron 14 mutation have no measurable CETP mass or activity, but those homozygous for the D442G mutation have only partial CETP deficiency and HDL-C levels that are less markedly elevated (39). Although these are the most frequent CETP mutations in Japan, several other mutations have been identified as well.
